Community Rehabilitation in BC: When Rehab at Home Beats the Clinic

Clinic or community? How community rehabilitation in BC works, who it helps most, and how ICBC, WorkSafeBC and insurers fund it.

Community Rehabilitation in BC: When Rehab at Home Beats the Clinic

You can do every exercise perfectly in a clinic gym and still struggle to carry groceries up your own stairs. That gap between "fine at the clinic" and "fine at home" is exactly what community rehabilitation in BC is built to close.

If you, a client or an employee is recovering from an injury, you will probably hear both options: rehab in a clinic, or rehab out in the community. Here is how they differ, who tends to benefit from each, and how the referral and funding side works.

The short version

  • Clinic rehab happens in a treatment room or gym. It suits people who can travel easily and mainly need hands-on treatment or supervised exercise.
  • Community rehab happens where life happens: your home, your workplace, the grocery store, the bus stop, or online.
  • Many recoveries use both. The question is which setting will move you forward fastest right now.
  • ICBC, WorkSafeBC, long-term disability insurers and Veterans Affairs Canada all fund community-based rehab in the right circumstances.

What community rehabilitation in BC actually means

Community rehabilitation brings the therapist to you instead of bringing you to the therapist. The work is built around your real routines, in the places you actually do them.

At Creative Therapy Consultants, our community rehabilitation team includes occupational therapists, kinesiologists, registered clinical counsellors, vocational rehabilitation consultants and rehab assistants. We see clients at home, in the community, at work and online, from 12 offices across BC and Alberta.

A community session might look like:

  • Practising getting in and out of the bath or car with an occupational therapist, then adjusting equipment or technique on the spot.
  • Building walking tolerance on your own neighbourhood route with a kinesiologist, instead of on a treadmill.
  • Breaking down a job task at your actual workstation before a return to work.
  • Rehearsing a grocery trip or a bus ride when anxiety after an accident makes those hard.

When clinic rehab makes sense

Clinic-based care is a good fit for plenty of people. If you drive, your pain is manageable, and you mainly need hands-on treatment or a structured exercise program, a clinic is efficient.

Clinics also have equipment that is hard to bring to a living room. For many people early in recovery, a clinic program is the right first step.

The trouble starts when progress in the clinic does not show up in daily life. If someone is "doing well in therapy" but still cannot manage a shift, a school run or a shower without help, that is often a sign the rehab needs to move into the real setting.

When community rehab is the better fit

Community rehabilitation tends to help most when:

  • Getting to a clinic is a barrier. Pain, fatigue, no licence after a crash, or living far from the nearest clinic.
  • The problem is functional. Cooking, bathing, childcare, driving or work tasks are the goal, not just strength or range of motion.
  • Home or work needs changes. An OT can assess the space and recommend equipment or modifications where they will actually be used.
  • Mental health is part of the picture. Fear of driving, crowds or returning to a job site is easier to work on in the real place, with support.
  • A return to work is on the table. See our workplace and employment services for how that support connects with employers.

Who pays for community rehabilitation?

Funding usually follows the injury.

Car crash: ICBC's first 12 weeks treatment page lists kinesiology among the treatments you can start without a referral or ICBC approval, using your Personal Health Number and claim number. ICBC also says it can fund occupational therapy, and that you should talk to your recovery specialist if your health care provider has recommended it. CTC provides ICBC Enhanced Accident Benefits rehab with direct billing.

Workplace injury: WorkSafeBC has occupational therapy providers on contract throughout BC, and CTC is a contracted provider. If you have a claim, Pain BC suggests asking your case manager about OT. More on our WorkSafeBC services page.

Other funders: long-term disability insurers, Veterans Affairs Canada, employers and lawyers also refer to us.

How to choose, and how to start

A simple test: write down the three things you most want to be able to do again. If they all happen in one place, like your home or your job, ask whether the rehab can happen there too.

Talk to your doctor, adjuster, case manager or recovery specialist about it. If you are a referral source, our what to expect page explains how a first visit works, so you can describe it to your client.

Frequently asked questions

What is community rehabilitation?

It is rehab delivered where you live, work and get around, rather than only in a clinic. Therapists work on real tasks in the real setting, at home, at work, in the community or online.

Does ICBC cover rehab at home?

ICBC funds a range of treatment after a crash. Kinesiology can start without approval in the first 12 weeks, and occupational therapy can be funded through your recovery specialist. Ask your provider whether they can see you at home and bill ICBC directly.

Can I do clinic rehab and community rehab at the same time?

Often, yes. Many people combine hands-on clinic treatment with community sessions that turn those gains into daily function. Your funder and care team can help plan the mix.

How fast will CTC reach out after a referral?

We contact the client within 1 business day of receiving the referral.
